Dear Mr. /Ms.
1. Bugando Medical Centre intends to apply parts of its funds towards the cost of proposed construction of outpatient clinics at Bugando Medical Centre, and intends to apply part of the proceeds of this fund to cover eligible payment under the contract for proposed construction of outpatient clinics at Bugando Medical Centre.
2. Bugando Medical Centre now invites proposals to provide the following consulting services: Proposed construction of outpatient clinics at Bugando Medical Centre. More details on the services are provided in the Terms of Reference.
3. A firm will be selected under Quality Cost based Method and procedures described in this RFP.
4. In addition to the Letter of Invitation, the RFP includes the following documents: 0 Section I : Information to Consultants (ITC)
0 Section 2 : Proposal Data Sheet (PDS)
0 Section 3 : General Conditions of Contract (CCC),
0 Section 4 : Special Conditions of Contract (SCC),
0 Section 5 : Proposal & Contract Forms

0 Section 7: Undertaking by Consultant on Anti – Bribery Policy / Code of Conduct and Compliance Program
5. Please inform us in writing or in electronic forms that provide record of the content of the communication, preferably by electronic mail, at the following address [email protected]/ [email protected], upon receipt
- That you have seen the Letter of Invitation and the RFP; and
- Whether you will submit a proposal alone or in association with any other Consultant.
6. A complete set of Request for Proposal (s) in English and additional sets may be purchased by interested Tenderer on the submission of a written application to the address given under paragraph 9 below and upon payment of a nonrefundable fee of TSH 100,000/=. Payment should be by Cash, payable to 0131051393600 CRDB, BMC HEALTH SERVICE FUND.
7. All RFP in one original and two copies properly filled in, and enclosed in plain envelopes together with the marked RFP No: PA/161/HQ/C/20222023/02, consultant for proposed construction of outpatient clinics at Bugando Medical Centre,and must be delivered to the address The Secretary Tender Board, Bugando Medical Centro, N4, AdmissistrationBlock, P.O BOX 1370 Mwanza.
8. The deadline for submission of RFP is Wednesday, 23/11/2022 at 10:00am Technical Proposal will be opened promptly thereafter in public and in the presence ofTenderer’s Representatives who choose to attend in the opening at the H4 VIP Hall at 10:00am.
9. Late proposal, portion of proposal, electronic proposal, and proposal not received, proposal not opened at the proposal opening ceremony shall not be accepted for evaluation irrespective of the circumstances.
