Physical Location:                 Namihodi Area, Mchinga Village, Lindi) Rural

                                                District in Lingl Region

Proposed change of name:     Fountain Petroleum Ltd – Mchinga Branch

Transferor:                            Abdillah! Kassimu Mandepe

Transferee:                           Fountain petroleum Ltd
