INVITATION FOR TENDER (IFT) N0: SMZ/FO118/ N-C/ NCB/ 2018-19/ 09

FOR

PROVISION OF MEDICAL FUND MANAGEMENT SCHEME TO ZURA STAFF AND THEIR DEPENDANTS

Zanzibar Utilities Regulatory Authority (ZURA) has set aside funds from its Budget for the financial year 2018/2019. it is intended that part of the proceeds of the funds will be applied to cover eligible payment under the contract for Provision of Medical Fund Management Scheme to ZURA Staff and their dependents.

Zanzibar Utilities Regulatory Authority now invites sealed bids from eligible and qualified bidders for Providing Medical Fund Management Scheme to ZURA Staff and their dependants.

The Scope of Services Include:

Medical Fund Management Scheme that covers all treatment as Inpatient in hospital includes the general wards, ICU & lCCU Covered Theatre, Surgical Operations, and Maternity etc.

Medical Fund Management Scheme that covers all treatment as Out-patient treatment include Various General &

Specialist Doctors, X-Ray Covered Laboratory Tests, optical 8: dental cover, Medicines Acute & Chronic, Maternity etc.

Foreign ln-patient treatment on referral for treatments unavailable in Tanzania and any other related services

The above named services constitutes one lot; and bidders must quote for all items and related activities specified in the lot.

Bidding will be conducted through the National Competitive Bidding (NCB) procedures specified in the Public Procurement and Disposal of Public Assets act No.1 1 of 2016 and are open to all Bidders as def1ned in the Act.

Interested eligible bidders may obtain further information from Secretary of Zanzibar Utilities Regulatory Authority (ZURA) Tender Board, P. 0, BOX 2238, Maisara Zanzibar Tel: 255-24-2238316 Fax: 255-24-2238317 email: [email protected]/www.zura.go.tz and inspect the Bidding Documents at the address given above from 9.00 AM 16:00 PM Monday to Friday except for public holidays.

Qualifications requirements include: (i) Must be a registered Service Provider (ii) must have provided similar services in the last five years. Additional details are provided in the Bidding Documents.

A complete set of Bidding Document(s) in English and additional sets may be purchased by interested Bidders on the submission of a written application to the address given under paragraph 4 above and upon payment of a non-refundable fee of Tanzania Shilling Two Hundred Thousands (TZS 200,000/=). Payment should be through People’s Bank of Zanzibar, Account No. 0404149000 payable to Zanzibar Utilities Regulatory Authority (ZURA).

All bids must be accompanied with a bid security declaration in an acceptable form.

All bids in one original plus three (3) copies, properly filled in, and enclosed in plain envelopes must be delivered to the address below at or before Wednesday 28 May, 2019 at 10: 00hrs am. Bids will be opened promptly thereafter in public and In the presence of Bidders’ representatives who choose to attend In the opening at the Conference Room, 2nd Floor of the Zanzibar Road Fund Building Maisara Zanzibar.

Late Bids, Portion of Bids, Electronic Bids, Bids not received, Bids not opened and not read out in public at the bid Opening ceremony shall not be accepted for evaluation irrespective of the circumstances.
